How much do visits cost?

First Office Visit $395 A comprehensive first office visit lasts approximately 60-90 minutes. You will fill out an online intake form that the doctor will review prior to your appointment. Some complex cases may require lab or imaging report review before your appointment. History taking and any necessary physical exam is performed during this visit. If needed, additional lab tests or imaging may be ordered. You will leave with an initial treatment plan tailored to your personal health goals.

Follow-Up Visit: $75-300 A return office visit typically lasts 15-60 minutes. This visit is used to discuss lab results, assess progress and determine any additional next steps.

IM Shots: $35 and up. A single shot of B12 or other nutrient is $35. A double shot or 2 different nutrients is $50. A triple shot or 3 different nutrients is $65.
Do you take insurance?

Elevate Health does not bill any insurance directly. Some insurance plans, like HMSA PPO, reimburse patients for consults. We can provide you with an insurance-coded receipt for reimbursement, but it is up to you to file the claim and understand your benefits. Elevate Health does not get involved in any insurance disputes under any circumstances. Please call your insurance provider to determine eligibility for reimbursement of visits and lab fees. Medicare, Medicaid and HMO plans do not reimburse for Naturopathic Care.
Do you validate for parking?

We validate up to 30 minutes in the Eaton Public Parking garage for patients being seen 30 minutes or longer. The Eaton Square garage charges $6/hour. There is a 15 minute grace period in the public parking garage, so quick supplement pick-ups and vitamin shots do not need parking validation. Metered street parking may be available at $3/hour.
What is your cancellation policy?

To run our clinic and maintain business operations, we require a 48-BUSINESS hours notice for cancellation. For example, if you contact the clinic over the weekend, when we are closed, for a cancellation on Monday, this would be a late cancellation. Cancellation fees depend on the visit booked. A 24-hour or less cancellation will be charged the full visit price. A cancellation over 24 hours but less than 48-business-hours is charged a fee of $25-50, depending on the service booked.
